You can fill out the form on your computer one of two ways.

Open up the PDF in your web browser and fill it out.

​On Windows, go to print. Once inside the print menu choose the option to save/print as a PDF (this option can be found under printer or destination, depending on your browser)

On Mac/Safari, go to file and click export as PDF and save the PDF.

Download the Benefit Profile PDF, fill it out and then "save as" a new PDF.

On a mobile device you can you use the free Adobe Fill app to complete the Benefit Profile.
